ADMISSION REQUIREMENTS


There are certain admission requirements for hospice care.The patient has a "life limiting" condition--due to a specific disease, a combination of diseases, or due to an undetermined cause, and clinical decline the last 6 months-- documented by physician or nurse evaluation, laboratory or radiological studies, or increased hospital or emergency department admissions.

The treatment goal is for relief of symptoms, not cure.


Coastal Hospice Care provides the following quality services to its patients:

• Regular nursing visits every week.
• Nurses available 24 hours per day, seven days a week.
• Certified nursing aides to attend to personal care.
• Social Workes to assist patients and family with community recourses.
• Spiritual and bereavement couselors
• Provisions of medicine, equipment and supplies related to their illness.
• Education for patients and loved ones on end-of-life issues.
• Volunteers to help patients and loved ones. 

Coastal Hospice Care will assist with other needs not listed above if necessary.

INTRODUCTION TO HOSPICE CARE


Coastal Hospice Care, Inc. was established in 2004 to offer a new approach to quality hospice care. We employ the finest dedicated professionals for your care. Our offices use state of the art computer systems to maintain accurate and timely information for your care.

Your care is provided in the most safe and comfortable environment — your home.

Coastal hospice provides the following professionals to provide care in you home:

Medical Directors, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Social Workers, Chaplains and Spiritual Advisors, Dieticians, Physical Therapists, Occupational Therapists, Speech Therapists, Homemaker Services and Volunteers.


HOSPICE CARE IN YOUR HOME


Hospice care is primarily provided in your home, where you can have the loving support of your family and friends. A large part of the hospice team’s role is to provide education and support to your caregivers.

Sometimes, patients may be unable to stay at home and must enter a nursing home or other facility. Coastal Hospice provides care to patients who reside in nursing homes or other types of facilities.

The primary objectives of Coastal’s hospice program are as follows:

To provide comprehensive, family-centered, coordinated, and competent care

To utilize an interdisciplinary group for care planning and delivery

To manage the patient’s pain and other symptoms

To maintain the patient in his/her own home or assure an inpatient setting as homelike as possible

To maximize patient/family autonomy and decision making and respect their beliefs, goals, and choices

To facilitate the patient’s and family’s capacity for reconciliation and closure

To provide bereavement services to family and significant others

To collaborate with other community resources to optimize continuity of care and comprehensive services.


THE TEAM APPROACH FOR YOUR CARE


At Coastal Hospice, we use the Interdisciplinary Group (IDG) as a team to provide you with care. No one-person can provide for all the needs of hospice patients and their families. Based on that assumption, Hospice care is provided by a “TEAM” of people with different specialties working together. This provides a more comprehensive and responsive form of care benefiting both you and your family. You and your family are the heart of the team, which also consists of the following people: Attending Physician, Hospice Medical Director, Clinical Director, RN/Case Manager,  Licensed Practical Nurse, Certified Nursing Assistant, Social Worker, Counselor, Chaplain, Volunteer Services Coordinator and Volunteer.
